Appointment

  • Note that if you cancel an appointment less than 24 hours in advance, you will be required to pay a $75.00 administrative fee. If you miss an appointment that had been confirmed, without a valid reason, you will be required to pay a $150.00 administrative fee.
  • Payment methods are: cash, check, debit or credit (Visa, Mastercard).
  • During the first appointment, we will ask you to fill out a medical questionnaire listing specific diseases you may have, medication you are taking and/or allergies you suffer from. This questionnaire may be completed online before your visit.

Surgeries

  • Major surgeries under general anesthesia performed in hospitals (CISSSME, Richelieu-Yamaska and Pierre-de-Saurel Pavilions) are covered by the RAMQ.
  • The day after the surgery, the nurse will contact you during the day to make sure you are controlling the pain appropriately and that you are not suffering from nausea or vomiting. If anything abnormal is noted by the nurse, she will immediately inform the surgeon that will call you back promptly.
  • One or several post-operation appointments will be scheduled depending on the type of surgery (minor administrative fees of 32.00$)
  • Minor surgeries are performed under local anesthesia with or without sedation (tablet, intravenously, nitrous oxide inhalation). The surgeon will explain in detail the various alternatives, as well as the pre-operation preparation and the required post-operation care (post-operation tip sheet).

Insurance

  • The administrative personnel will provide you with receipts for insurance purposes. SAAQ, CNESST and IVAC documentation will be completed, as well as the medical certificates attesting your leave.

In order to facilitate your work and in the interest of seeing your patient as soon as possible, please fill out the attached document, attach photos and/or x-rays (according to the suggested formats), sign it (with your license number) and return it to us by email.
